Forklift Safety: The Incidents That Actually Happen, and Why

Forklift incidents cluster into a small number of recurring patterns. None of them are exotic, all of them are well understood, and nearly all of them are prevented by the same handful of practices. Worth going through the actual mechanisms rather than the poster, because understanding why something happens is what changes behaviour.

Tip-Overs

The most serious category, and the one where the instinctive reaction is the fatal one.

Lateral tip-over usually comes from turning too fast, turning with an elevated load, or operating on a slope across the gradient. A counterbalance truck is stable while the combined centre of gravity of truck and load stays inside the triangle formed by the front wheels and the rear axle pivot. Raising a load raises that centre of gravity; turning adds lateral force. Do both at once and the margin disappears quickly.

Longitudinal tip-over comes from overloading, from a load center further out than the truck is rated for, or from braking hard with a raised load.

The critical point: in a tip-over the operator must stay in the seat, hold on, brace their feet and lean away from the direction of fall. The overhead guard is designed to protect them. Jumping is what turns a survivable incident into a fatal one, because the operator lands where the truck is going. This is counter-intuitive under stress, which is exactly why it has to be trained rather than assumed.

Seatbelts matter for the same reason: they keep the operator inside the protective structure.

Pedestrian Strikes

The most common serious-injury category in mixed environments, and the one most affected by layout rather than by operator skill.

Contributing factors are consistent: blind corners, loads blocking forward visibility, pedestrians taking shortcuts through truck routes, reversing without a clear view, and noise levels that mask an approaching truck.

What actually works:

  • Separate routes where possible. Physical separation beats every behavioural control.
  • Marked walkways that are actually convenient, because inconvenient ones get ignored.
  • Mirrors at blind corners and at aisle ends.
  • Travel with the load low and, where visibility forward is blocked, in reverse — with the operator looking in the direction of travel.
  • Horn at intersections and blind corners, consistently rather than occasionally.
  • Pedestrian awareness training, not just operator training. The people walking need to understand what the truck driver can and cannot see.

Falls From Elevated Forks

People still ride on forks, and they still fall. Forks are not a work platform, and elevating a person on them is prohibited.

Where people genuinely need to work at height from a truck, the answer is a properly designed and secured work platform, with the operator remaining at the controls throughout. Order pickers are the other answer where the work is picking at height — and on an order picker, a body harness with the lanyard attached to the designated anchor point is required, not optional.

Dock Incidents

A distinct category with its own causes, and a serious one because the consequences are severe.

  • Trailer creep — the trailer moves away from the dock as the truck drives in and out. Restraints or chocks prevent it; nothing else reliably does.
  • Early departure — a driver pulls away while a truck is inside. Communication systems and physical restraints address this; verbal agreement does not.
  • Falling off the dock edge — particularly reversing, and particularly where an empty dock position is not visibly marked.
  • Unstable trailer floors — worth checking before driving a loaded truck onto one.
  • Dock plate failure — capacity has to cover truck plus load, and plates need checking.

Load Handling Failures

Loads falling from forks, rack being struck, product damaged. Causes are mundane: forks not fully under the load, load not centred, travelling with the load raised, damaged or mismatched pallets, and stacking beyond stable height.

Rack strikes deserve particular attention because the damage is frequently not reported. A struck upright that stays in service under load is a genuine collapse risk, and the person who hit it is often the only one who knows. A no-blame reporting culture is worth more here than any amount of signage — the objective is that the strike gets inspected, not that someone gets disciplined.

The Daily Check Prevents More Than People Think

OSHA requires a pre-shift examination, and most of the mechanical failures we are called to would have been caught by a real one: hydraulic leaks, chain wear, fork cracks, brake travel, steering play, a parking brake that will not hold.

The failure mode is not that operators cannot do the check. It is check sheets signed in batches at the end of the week, which is documentation that the hazard was formally overlooked rather than evidence it was managed.

What Changes Outcomes

  1. Certification that is real, current and truck-type specific. Not a card issued once for a truck the operator no longer runs.
  2. Separation of trucks and pedestrians wherever the layout permits it.
  3. Seatbelt use, enforced. The single highest-value behaviour in the most serious incident category.
  4. Travelling with loads low. Raises the stability margin on every move.
  5. A daily check that actually happens.
  6. Damage reported without blame, so rack strikes get inspected rather than hidden.
  7. Refresher training after incidents and near misses, which the standard requires and which is easy to skip.
